This form needs to be 100% accurate and have all request noted in the highlights section. - Start Date should be on or very quickly after the Order Start Date listed.
- All RTO should be listed on the cover page, must be less than 7 days.
- RTO will not be approved in the first week of assignment or holidays.
- All facilities the traveler is open to working in should be listed on the submission.
- If interested in any location, please indlude what states (I.E. Any AZ, Any CO, Any state, etc.)
- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
- Please let us know if the traveler withdraws or is no longer available.
Requirement description :
- Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission
- 3+ years minimum of current Oncology experience - REQUIRED
- Must have ONS/ONCC certification - REQUIRED
- Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
- No License Hits - REQUIRED
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
- Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**
