Job Summary:  
A hospice clinical triage registered nurse plays a critical role in assessing and managing the needs of patients who are receiving hospice care. Other responsibilities typically include:
  • Assessment: Conducting comprehensive assessments of patients to determine their current medical condition, symptom management needs, and overall level of comfort.
  • Triage: Fielding incoming calls and needs. These calls are varied in scope from simple med approval to education. Prioritizing patient needs based on the severity of symptoms and coordinating appropriate interventions.
  • Symptom Management: Providing guidance and support to patients and their families on managing symptoms such as pain, nausea, and shortness of breath. This may involve recommending medications, therapies, or other interventions to alleviate discomfort.
  • Communication: Serving as a point of contact for patients, families, and other members of the healthcare team to address concerns, provide education, and facilitate communication regarding the patient's care plan.
  • Coordination: Collaborating with other healthcare professionals, including physicians, social workers, and therapists, to ensure that the patient's care needs are met holistically and efficiently. Act as a resource to nurses as needed; immediate availability for nurses in the field to defer to another resource in the office if they are bedside.
  • Education: Encompasses a range of topics aimed at providing knowledge and skills to healthcare professionals, patients, families, and the community about end-of-life care, palliative care, and the philosophy of hospice.
  • Documentation: Maintaining accurate and up-to-date medical records, including assessments, care plans, and communication with patients and families. Fall reporting. Review of charts flagged by the risk stratification tool in place with PCPs to recommend for evaluation and hospice placement.
  • Support: Offering emotional support and counseling to patients and their families as they navigate end-of-life care decisions and cope with the challenges of terminal illness. Contribute to a culture of helping, making staff feel more supported.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $80,000 – $98,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
