Overview
We are seeking a compassionate and experienced Registered Nurse to join the Home Health care team serving Western Pennsylvania and surrounding areas. This weekend position focuses on providing high-quality, patient-centered care and managing individualized care plans for patients in the comfort of their homes. The ideal candidate is highly organized, dependable, and skilled in managing multiple patient visits while maintaining accurate and timely clinical documentation.
Key Responsibilities
Manage and deliver comprehensive patient care plans in coordination with the healthcare team
Provide in-home nursing care, ensuring patient safety, comfort, and adherence to treatment plans
Support patients across Western PA through compassionate, evidence-based care
Spend approximately 20% of the workday traveling to and from patient homes
Maintain accurate, up-to-date documentation within the electronic medical record (EMR) system
Meet all required charting deadlines and ensure compliance with regulatory and organizational standards
Self-manage time, schedule, and daily caseload effectively
Schedule and Shift Details
On-call Friday, Saturday, and Sunday from 5:00 p.m. to 10:00 p.m.
Eight (8) units on Saturday and eight (8) units on Sunday
Two required holidays per year
Travel
Local travel required throughout Western Pennsylvania and nearby areas
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience
Experience with clinical documentation and electronic medical record (EMR) systems (e.g., Practice Partner EMR)
Current certifications in the following:
American Red Cross Advanced Life Support (ARC ALS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Reliable transportation and willingness to travel locally
Preferred
Experience in home health, clinical informatics, or quality improvement
Strong organizational and time-management skills
