Overview
We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our client's growing team in Chicago, Illinois. This role focuses on delivering skilled, patient-centered care in the comfort of patients' homes, helping them maintain independence and improve overall well-being. The ideal candidate will bring clinical expertise, strong communication skills, and a genuine commitment to providing high-quality care.
Key Responsibilities
Deliver professional nursing care and perform comprehensive patient assessments in home settings
Develop individualized care plans in collaboration with physicians and the healthcare team
Educate patients and families on treatment plans, medications, and post-care instructions
Monitor patient progress and adjust care as needed to achieve optimal outcomes
Maintain accurate and timely documentation in accordance with regulatory standards
Ensure effective communication and coordination among care team members
Schedule and Shift Details
Monday through Friday, flexible schedules with no long shifts
Minimal weekend shifts per year
This is a Pay Per Visit position
Travel
Local travel required within the Chicago area (travel reimbursement provided)
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid driver's license, reliable vehicle, and current auto insurance
Preferred
Prior experience in home health or community-based nursing
Strong assessment, communication, and time management skills
